Camp Registration

I hereby grant permission for the player named above to participate in instructional baseball lessons. I understand that baseball involves inherent risks of physical injury, including but not limited to being struck by balls, bats, or other equipment, and physical exertion. I knowingly and freely assume all such risks, both known and unknown.

In consideration of receiving instruction, I hereby release, waive, and hold harmless the instructor(s), facility owners, and affiliates from any and all claims, liabilities, or demands for personal injury, property damage, or wrongful death arising out of or in connection with these lessons.

I certify that the player is in good physical health and capable of participating in strenuous physical activity. In the event of a medical emergency, I authorize the instructor to obtain necessary medical treatment for the player if a parent/guardian cannot be reached immediately.
